Popular Form
We are now going to build a popular simple form that asks for a visitors name, e-mail, address, telephone number, subject matter, city and state. In addition I have added a button that will allow a visitor to clear the form if they made a mistake and need to start over.
The form here may not look good but again that is a design issue. This is designed to show you a form with several options.
You can add more to the form if you need to have a country list or other items.
